Patriot Golf invites you to the 9th Annual Jake Hoffman Memorial Golf Charity Tournament
Proceeds benefit UMASS Amherst Isenberg School HTM program, Huntington’s Disease Youth Organization and our Acts of Kindness Program.
When: Monday, October 9th (COLUMBUS DAY) 2023
Time: Registration will begin on 10/11 at 8:30am; tournament starts at 10:00am Breakfast will be served starting at 8:30 to all Golfers
Where: Brookmeadow Country Club, 100 Everendon Rd. Canton, MA 02021
Price: $150 per person (Teams of 4; Team Cost: $600)

The Scholarship
Proceeds from this golf tournament provide financial assistance to UMASS Amherst Isenberg School HTM program, Huntington’s Disease Youth Organization and our Acts of Kindness Program which provides support to local families and organizations in need. Our goal is to keep Jake’s light shining through acts of kindness.
